Please read and decide about the following Data Use Agreement:
SCRIPT Data Use Agreement v1.0
The dataset presented here is part of the CarpeDiem v1.1 dataset [DOI]
selected for demonstration purpose only.
If I am granted access to the dataset:
1. I will not attempt to re-identify any individual in SCRIPT restricted data.
2. I will exercise all reasonable and prudent care to avoid disclosure of the identity of
any individual referenced in SCRIPT restricted data in any publication or other
communication.
3. I will not share access to SCRIPT restricted data with anyone else.
4. I will exercise all reasonable and prudent care to maintain the physical and
electronic security of SCRIPT restricted data.
5. If I find information within SCRIPT restricted data that I believe might permit
identification of any individual, I will report the location of this information
promptly by email to script@northwestern.edu, citing the location of the specific
information in question.
6. I have requested access to SCRIPT restricted data for the sole purpose of lawful use
in Infectious Disease research, and I will use my privilege of access for this purpose
and no other.
7. I have completed a training program in human research subject protections and
HIPAA regulations (e.g., The Collaborative Institutional Training Initiative [CITI
Program] courses “HIPAA, Security and Privacy” and “Biomedical Research”) and
attest that the certificate of this training is not expired.
8. If I openly disseminate my results, I will also contribute the code used to produce
those results to a repository that is open to the research community.
9. This agreement may be terminated by either party at any time, but my obligations
with respect to SCRIPT data shall continue after termination.
10. If I am unsure about the applicability of any of these clauses, I will contact the
SCRIPT team at script@northwestern.edu before proceeding.
Publication requirements:
1. For any publication leveraging SCRIPT data I will include the following
acknowledgement:
The SCRIPT data used in this work was generated by NIAID grant 5U19AI135964.

2. If the publication is accepted, I will send a copy of the publication with description
of how I used the data to the SCRIPT team at script@northwestern.edu.
